We offer two distinct plans for Direct Primary Care, whether you’re looking for Essential care or Premier coverage, take control of your health TODAY and choose the plan that’s right for you.

*No long term contracts or commitment (except for weight management plans), but we do ask for a 30 day notice for all member cancelations.

Premier Membership

Our Premier membership offers a level of care that is truly exceptional. Whether you require management of your chronic medical condition(s), need medication management, or you simply want access to a level of care that goes above and beyond, this option is for you.

Includes unlimited visits and direct access to your care team.

Introductory Pricing:

  • Adult (> age 19): $125.00/per month
  • Each child with adult membership: $50.00/per month
  • Each child without an adult membership: $85.00/per month
  • Family Max:  $325.00/per month (spouses and children <19 years old)
  • College student with valid student ID: $75.00/per month

Essential Membership

With our Essential membership, you can expect healthcare services delivered with the utmost professionalism and dedication. This package is tailored for those in excellent health and do not have recurring prescription medications that require management. This plan provides our patients with the essential care they need to stay healthy and feeling their best.

Introductory Pricing:

  • $550.00/per person/per year
    • Must be paid in full upon enrollment. 
    • Additional appointments above 3/per year: $200/per visit.

** Upgrade to our Premier Membership status at anytime and any unused visits/balance can be applied to your membership.

*We require a one-time, non-refundable enrollment fee: $100/Adult and $50/Child. 

*An enrollment fee allows our team to verify that a correct billing method is on file, ensures all legal documents are signed including a release of information, and gives our team the time necessary to ensure accurate and complete transfer of medical information into your chart in our new system.
