Our membership-based model is available to those that are enrolled in the Premier membership, as an add-on special pricing. This means that you know exactly what you're paying for, with no hidden fees or surprises. 

*As a chronic health condition, obesity is a challenging issue that impacts your physical and emotional well-being.  It also takes dedicated time from both your clinician and you to reach and sustain the goals we set.  Therefore at this time, this plan is only available to our Premier DPC members at the introductory additional add on pricing.

Initial Phase: 

*Requires a minimum 3 month commitment

  • +$150/per person/per month added to your Premier Membership

    • Initial 60-90 minute comprehensive visit & exam.

    • INITIAL comprehensive obesity bloodwork & lab draw INCLUDED. **All follow up lab work at our LOW-COST/WHOLE-SALE PRICE (if paying CASH to our office), LAB draw included if drawn in our office.

    • Check-in monthly for accountability weigh-ins with your care team.

    • Unlimited visits with your clinician

    • Plans tailored to you.

    • Access to our private Facebook support group *Coming Soon

    • Options to discuss compounded GLP-1/GIP agents through a specialty pharmacy- *Cash-Based & additional cost.

Maintenance Phase:

Congratulations!  you have met your goal and are on the way to your best self! We want you to sustain your goals to ensure they are long-lasting.

  • +$50/per person/per month added to your Premier Membership

    • Continue visits with your primary team as often as you need, with unlimited visits.

    • Continue to have access to our private Facebook support group *Coming Soon

    • Continue to have access to any/all obesity medication management.

    • ONCE A YEAR obesity lab work & lab draw included.
